NEWS2 Calculator NHS

This independent UK NEWS2 calculator follows the Royal College of Physicians (RCP) National Early Warning Score 2 framework for adults with acute illness. NEWS2 is used to standardise recognition and response to clinical deterioration. It does not replace clinical judgement or local escalation policy.

NEWS2 total (Scale 1 scoring): 0

Safety: For SpO2 Scale 2, do not use the displayed total as a clinical NEWS2 total. Scale 2 has a different saturation scoring table and should only be used when a qualified clinician has documented an 88–92% target, usually in hypercapnic respiratory failure. Use the official RCP chart/local clinical system.

What NEWS2 measures

NEWS2 scores six physiological parameters: respiration rate, oxygen saturation, systolic blood pressure, pulse, consciousness/new confusion and temperature. Supplemental oxygen adds 2 points. Higher aggregate scores indicate greater physiological disturbance and the need for appropriate clinical escalation.

NEWS2 thresholds and triggers

The RCP framework uses an aggregate NEWS2 score together with any single parameter scoring 3. Clinical response depends on the total, individual extreme parameters and local escalation protocols. A score is an aid to assessment, not permission to delay review when a patient appears seriously unwell.

SpO2 Scale 1 and Scale 2

Scale 1 is used for the majority of patients. Scale 2 is specifically for patients whose target oxygen saturation has been set at 88–92% following a clinical decision, commonly because of confirmed hypercapnic respiratory failure. The RCP says that decision must be recorded. Because Scale 2 scoring changes according to both saturation and supplemental oxygen, this page deliberately does not approximate it.

Worked example

For example, a patient with respiration 18/min, SpO2 97% on air using Scale 1, systolic BP 125 mmHg, pulse 82/min, alert consciousness and temperature 37.0°C scores 0 on each parameter, giving NEWS2 0. A normal score does not rule out serious illness if there are other clinical concerns.

Important 2026/27 context

NEWS2 remains embedded in NHS deterioration pathways for adults. NHS England’s current Managing Deterioration Safety Improvement Programme describes implementation of NEWS2 in acute and ambulance settings, while paediatric deterioration uses the separate national PEWS approach.

Limitations

NEWS2 is designed for adult acute illness and should be used by appropriately trained healthcare staff within clinical pathways. It is not a self-diagnosis tool. It is not the national paediatric early warning score, and pregnancy or specialist settings may use different or additional systems. If a patient is acutely unwell, urgent clinical assessment should not be delayed to calculate a score.

FAQs

What does NEWS2 stand for?

NEWS2 means National Early Warning Score 2, the RCP system for standardising assessment and response to acute illness.

What observations are used in NEWS2?

Respiration rate, oxygen saturation, systolic blood pressure, pulse, consciousness or new confusion, and temperature are scored; supplemental oxygen is also recorded and scored.

When should SpO2 Scale 2 be used?

Only after a clinical decision has set a target oxygen saturation of 88–92%, usually for confirmed hypercapnic respiratory failure, and the decision has been documented.

Is NEWS2 used for children?

NEWS2 is an adult deterioration score. NHS England has a separate national Paediatric Early Warning System for children.

Disclaimer: This independent educational calculator is not an NHS clinical system. Healthcare professionals should use the official RCP NEWS2 chart and their organisation’s current escalation policy.
